Metrowest Forms Page

There are a number of different forms you can access from our web site. To make it easier, we have provided links to all the forms here, organized according to subject.

Cooperative Purchasing

Metrowest members may enroll in our MARLS Materials and Supply Cooperative. This is a cooperative that provides you discounts when ordering books and supplies. You will need to print these forms, fill them out, and send them back to us.

• Participation Form
  This form sks for information that will be provided to our vendors.
• Pledge Form
  This form asks for budget information.
• Material and Supply Cooperative Feedback Form.
  This is a form you can fill out online to submit your complaints, compliments, and questions about the Materials and Supply Cooperative.

Delivery Forms

These are forms associated with our Delivery Service.

• Routing Slips for All Regions (pdf)
  This is a document you can print that contains blank delivery slips for all the regions.
Metrowest Routing Slips: Four on a Page (pdf) Regional Routing / Delivery Slips
This is a document you can print that contains blank delivery slips for the Metrowest Region.
• All Libraries Routing / Delivery Slips
  This is a PDF document that contains preprinted delivery slips for all the libraries on the Metrowest delivery system.
• Delivery Claim Form
  This is a printable form to claim items damaged during delivery.

Membership Forms

These are forms you will need to print, fill out, and return to us for Metrowest Regional Library System Membership.

• Metrowest Membership Agreement
  This is a printable form where you agree to participate as a member of the Metrowest Regional Library System.
• Certification of Membership Eligibility
  This is a Word document you can download and print, certifying that you meet all eligibility requirements for membership.

Metrowest Committee Forms

These are forms you can fillout regarding a Metrowest committee.

• Committee Volunteer Form
  This is an online form where you request to participate on a committee.
